Insurance Claim Investigations
Investigative Services Unlimited Group provides objective, documented investigations for insurers, special investigation units, third-party administrators, self-insured employers, businesses, and defense counsel. We focus on assignments where professional fieldwork, evidence development, surveillance, interviews, and organized reporting can materially assist claim evaluation or litigation.
Priority Insurance Investigation Services
The following assignments generally require the greatest investigative skill, coordination, documentation, and case management:
- Complex SIU and suspected-fraud investigations: Multi-subject inquiries involving inconsistent statements, concealed relationships, staged events, questionable documents, suspicious providers, repeated claims, or coordinated activity.
- Workers’ compensation investigations: Lawful surveillance, activity checks, employment or business research, witness interviews, workplace inquiries, and documentation related to reported injuries or functional limitations.
- Commercial property and major-loss investigations: Scene follow-up, ownership and vendor research, witness development, business-activity verification, documentation review, and investigation of disputed theft, fire, water, equipment, inventory, or interruption claims.
- Staged automobile accidents and organized claims: Relationship research, participant and witness interviews, address verification, prior-claim indicators, business affiliations, scene investigation, and timeline development.
- Disability and long-term benefit investigations: Carefully planned surveillance, activity documentation, public-source research, business or employment indicators, and factual reporting without medical conclusions.
- Liability and catastrophic-injury investigations: Early scene documentation, witness identification, video-source inquiries, background research, event reconstruction, and litigation-focused evidence preservation.
- Provider, vendor, and billing-pattern inquiries: Corporate and licensing research, location verification, ownership or referral relationships, public advertising, witness information, and other lawful facts relevant to questioned services or invoices.
- Attorney-directed insurance litigation support: Witness location and interviews, exhibit and timeline development, surveillance review, evidence organization, records research, and preparation of clear investigative reports for counsel.
How an Assignment Begins
We review the claim file, reported loss, prior statements, available photographs or video, policy questions identified by the client, litigation status, deadlines, and specific inconsistencies requiring investigation. A focused plan is then developed around the strongest factual questions. Complex matters may be handled in phases so the client can evaluate early findings before authorizing additional work.
Investigation Methods
- Surveillance: Proportionate observation from lawful vantage points, supported by chronological logs, photographs, and video references.
- Recorded and fact interviews: Claimants, insureds, witnesses, employers, coworkers, neighbors, vendors, and other sources contacted within the authorized scope.
- Scene and location investigation: Conditions, access, occupancy, cameras, witnesses, business activity, property, vehicles, and relevant physical details.
- Background and relationship research: Court, corporate, property, licensing, address, lien, judgment, business, and public online sources used for a lawful purpose.
- Claim and evidence review: Applications, statements, estimates, invoices, photographs, correspondence, prior investigations, recorded material, and documents supplied through proper channels.
Deliverables for Insurers and Counsel
Depending on the assignment, the client may receive an investigative report, claim chronology, interview summaries, surveillance logs, photographs, video references, subject or business profiles, relationship charts, source indexes, scene documentation, and recommendations for additional factual follow-up. Reports explain what was confirmed, contradicted, or unresolved and identify investigative limitations.
Structured and Ongoing Assignments
We are available for single assignments, phased complex investigations, litigation matters, and ongoing authorized claim-support work. Longer engagements can include scheduled status reports, defined investigative milestones, coordinated field coverage, and a central point of contact for counsel, claims professionals, or SIU personnel.
Professional Boundaries
We do not decide coverage, compensability, disability, impairment, medical necessity, employment discipline, or guilt. We do not trespass, harass, unlawfully track or intercept communications, access protected records without authority, or guarantee that surveillance will document a particular activity. Our role is to develop and report facts objectively.
